Thank you for your interest in enrolling your child in the International Violin Institute. This form is required to complete registration and confirms your authorization for the collection, use, and processing of your child’s information for scheduling lessons, providing services, and processing payments, in accordance with our policies.

Verifiable Parental Consent

By checking this box, you agree and consent to the following:

1. You are verifying your consent to our collection, use, and disclosure of personal information from or about your child (further described in our privacy policy which you acknowledge you have reviewed), as required by the Children’s Online Privacy Protection Act (COPPA). This includes name, address and other information we and the businesses we work with need to schedule lessons, provide Services, and process payments.
2. You consent to our contacting you at the email, phone(s), and mailing addresses you have provided. You may revoke this consent or refuse further collection or use of your child’s information at any time by contacting us at info@InternationalViolinInstitute.com. If you do not accurately complete and return this verifiable consent form, we will delete your online contact information from our records.
3. You acknowledge that you are the parent or legal guardian of a child who is, or may become, enrolled with the International Violin Institute, that you are accessing our website to explore enrollment of your child or to ensure your child receives our services.

Release of Images

I authorize the Institute to publish on the Web or Social Media or otherwise release any pictures, live streams, or recordings of my child as a participant in the Institute and I understand that these may be used for promotional purposes.
Please check box if you do not consent to the Release Of Images Statement in our Enrollment Form. This box, if left unchecked, means that you do consent to any publications or releases.

Note: You do not need to consent in order for your child to participate in the Institute.
